ACADEMIC PLANNING AND COORDINATION

Once a child has received an assessment either from the Center or elsewhere, the Center can follow the child by providing academic planning and coordination.  This service consists of educating families so that they thoroughly understand their child's diagnosis, assisting them to implement the recommendations made, and remaining available to them to help coordinate services for their child for a full year.  Services might include attending school meetings, assisting them to access community services, providing educational materials about the child's diagnosis, meeting with the child and his parents to develop organizational and study skills plans, assisting to fill out application forms, etc.

THE APPLICATION PROCESS

To access academic planning services when an evaluation has been completed outside of the Center, the family need only call the Office Manager of the Center. She will complete intake information and send out an application packet. The packet contains releases of information, which the family sends to treating physicians and school settings, and a developmental questionnaire. When the Center has received all of the medical and academic information requested, and the family has returned the developmental questionnaire, Melody Allen Lenkner, the director of the Center, will summarize the information and meet with the family in an initial academic planning. This session  usually lasts an hour to an hour and a half. At that time the family and Melody will determine  the child's primary academic planning  needs and develop a Success Plan. Melody and Jolene, who are pictured below, provide these services for our families.

Children who have been evaluated at the Center receive academic planning services automatically as part of their evaluation.
